Student or Associate - Form
- Associate Application - Applicant form to become a CCAA Associate
Graduate and Clinical Member - Forms
- Registered Member Application - Applicant form to become a CCAA Graduate or Clinical member
- For NSW only at this stage, complete both forms below to join CCAA and PACFA simultaneously. Most applicants complete only 4 pages, and avoid considerable doubling up of paperwork.
- CCAA Intern, Graduate, Clinical Application - NSW applicants only
- PACFA Register Application - 4 pages of core evidence of training
Supervisor-signed Counselling/Supervision Logs - Certified Transcripts
It is a PACFA and CCAA requirement that ALL applicants who are counselling supply:
- Proof of their counselling and supervisor hours duly signed by supervisors. This means logs of some description, eg Course Counselling/Supervision Logs, the CCAA PD and Supervision Log or the PACFA Supervision and Client Contact Log.
- Certified copies of transcripts of counselling training (and certified copies of certificates if available). See Who can Certify Course Transcripts and Certificates?at the bottom of the Membership Forms page.

Who can Certify Course Transcripts and Certificates?
Course Transcripts and Certificates need to be certified. This can be done one of the following ways:
1. Your College Registrar’s Office
2. Take your originals along with a photocopy to anyone who is currently employed as:
- an accountant - member of the Institute of Chartered Accountants in Australia, or the Australian Society of Certified Practising Accountants, or the National Institute of Accountants, or the Association of Taxation and Management Accountants or Registered Tax Agents
- a bank manager, but not a manager of a bank travel centre
- a credit union branch manager
- a commissioner for declarations
- a barrister, solicitor or patent attorney
- a police officer in charge of a police station, or of the rank of sergeant and above
- a postal manager
- a principal of an Australian secondary college, high school or primary school.
3. Take your originals along with a photocopy to a Justice of the Peace in Australia with a registration number.
